Providence Health & Services RN-Field Nurse Elder at Home in Newberg, Oregon

Description:

Providence is calling a Registered Nurse - Field Nurse Elder at Home (0.6 FTE, Days) to Providence Newberg Medical Center in Newberg, OR.

In this position, you will:

  • Be an integral and clinically proficient member of the Elder at Home interdisciplinary care team.

  • Provide targeted urgent care assessment and related skilled nursing interventions in the home/community setting for the most medically fragile PHP Medicare Advantage, at-risk patients and their families/caregivers as part of ongoing care coordination and transitions of care management and to prevent care escalation and/or facilitate appropriate transition to a higher level of care.

Qualifications:

Required qualifications for this position include:

  • Current, unencumbered state of Oregon RN license

  • American Heart Association Basic Life Support (AHA-BLS) for Health Care Provider completion within 1 month of hire

  • Valid driver’s license

  • Driving record consistent with GOP Safe Driving Policy 330 Incident Point Matrix at time of hire

  • 2 years of clinical medical-surgical, acute care or long-term care experience that includes:

  • 1 year of community-based/ambulatory care and

  • 1 year of in care of the frail elderly

Preferred qualifications for this position include:

  • Bachelor’s Degree in Nursing

  • Current recognition by a national professional organization in case management:

  • Case Management Society of America (CSMA) – CCM credential

  • American Case Management Association (ACMA) – ACM credential

  • American Nurses Credentialing Center (ANCC) – RN-BC

  • 1 year of care management of diverse patient populations

  • Home health, hospice, palliative care, managed care, or skilled nursing facility background

  • Working knowledge of Medicare criteria for skilled nursing services or home health/medical equipment services
