Providence Health & Services Mental Health RN - Home Health in Newberg, Oregon
Providence is calling a Mental Health RN - Home Health (0.6 FTE, Days) to Providence Home Health Yamhill in Newberg, OR.
In this position, you will have the following responsibilities:
Provides quality care to patients in their homes under the direction of a physician.
Functions as a consultant in the nursing assessment of patients who are managed by other disciplines in the department and coordinates patient care activities with other members of the health care team.
Responsible for the assessment, treatment, and care of patients of all ages including adults and geriatric.
Responsible for monitoring medical, physical and psychiatric care needs of assigned patients in Home Health.
Required qualifications for this position include:
Current unencumbered Oregon Registered Nurse License.
Bachelor’s Degree in Nursing from an accredited school of nursing.
Current BLS (Basic Life Support for Health Care Provider) or ACLS (Advanced Cardiac Life Support) required within 30 days of hire.
3-5 years’ experience in acute care or community mental health setting.
Valid driver’s license and reliable transportation
In compliance with the Providence Health & Services-OR Human Resources policy #302 “Background Checks”, a criminal background check is required for this position, based on assigned duties.
Driving record consistent with GOP Safe Driving Policy 330 Incident Point Matrix at time of hire.
Preferred qualifications for this position include:
Recent Home Health experience
Point of Care experience
Community mental health experience and geriatric experience preferred.
ANCC Psychiatric Certification preferred.
